Cancellation Policy
I understand that if I cancel within 12 hours of my scheduled appointment , I am subject to pay 50% of the scheduled service. If I do not show up to my scheduled appointment without any notice, I am subject to pay 100% of the scheduled service. I allow “BLASE SKIN STUDIO” to charge my credit card on file if I violate these terms.
If I am under the age of 18, a parent or guardian must sign this form on my behalf.
This agreement will remain in effect for the service and for all future services conducted by “BLASE SKIN STUDIO” from the date of the signed form.
